Alcoholic Beverage Waiver Permit
This Alcoholic Beverage Waiver Permit is required for all individuals, groups, or organizations that plan to serve alcoholic beverages at a function at the Fertile Community Center.
The undersigned understands that no alcoholic beverages may be sold or consumed in the facility or on the premises without an alcoholic beverage waiver permit and that no alcoholic beverages may be sold, given to, or consumed by anyone under 21 years of age anywhere in the facility or on City property: and that he/she is responsible for seeing that all Local, State and Federal laws as to the age restriction for alcoholic beverages is followed: and will not provide any alcoholic beverages to minors nor allow any other guests or attendees at the event to provide any alcoholic beverages to minors.
The City of Fertile reserves the right to require the presence of police officers (or a staff person), at renters expense, at any event where alcoholic beverages are served unless there is evidence that the event is of a nature that will not require such protection, as determined by the City Manager whose decision shall be final. If a police officer (or staff person) is required, both have the authority to eject individuals for rowdy, destructive, or inappropriate behavior, to contact law enforcement to have offenders arrested if public laws are thought to have been violated, and to cancel and empty the facility if he/she determines the protection of the facility or others may be in jeopardy.
The undersigned understands that he/she is responsible for the conduct and behavior of the guests attending the event and for any damages, accidents, or injuries to themselves or others by persons becoming intoxicated: and further agrees to indemnity and hold harmless the City of Fertile in any lawsuit which is filed against it arising out of or as a result of said event where alcohol was served.
